• Payments will be deducted automatically from the designated credit card each week for the specified period and amount provided.
• While enrolled in Recurring Payments I may make an additional payment at any time
• If the above noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day.
• If my request is approved, my credit card will be enrolled in Recurring Payments. Once I am enrolled, if I require any changes to the Recurring Payments information I must contact the House Arrest office by calling 702-671-0821.
• If I do not update my account and my designated credit card is no longer valid and my payment is returned for an invalid account number, Recurring Payments will be TURNED OFF and I will have to re-enroll.
• I agree to the additional fee of 2.65% per recurring transaction.

The convenience fee is collected by a third party, will be represented on your credit card statement as a separate transaction.

I authorize CC LVMPD House Arrest to initiate a recurring electronic payment in the amount specified from the credit card on this record. I also authorize my financial institution to honor this electronic payment. This Authorization shall remain in effect until the date specified. The Authorization on file with the latest effective date supersedes and replaces any and all previous authorizations. I certify that I am an authorized user of this credit card and will not dispute these scheduled transactions with my credit card company; so long as the transactions correspond to the terms indicated in this authorization form.
